What is institutional knowledge, and why do companies lose it?
Institutional knowledge is everything a company knows that is not written down: why decisions were made, what was promised, what failed before. Companies lose it because it lives in heads, and heads leave.
Every departure is an uncontrolled deletion of company memory. Every reorganization scatters it further. The wiki was supposed to fix this and never did, because writing things down is work and keeping them current is more work.
Heidi's answer: capture it as a side effect of work already happening, meetings, chats, tickets, and keep it alive automatically. Knowledge stops walking out the door.
